Patient Information

Your First Visit

Your consultation visit at Erickson Orthodontics is complementary, at no charge to you. We will take an x-ray and digital photos of your teeth and face. After reviewing these, Dr. Michael J. Erickson  will listen to your concerns and desires, and he will develop a plan for your orthodontic treatment that best fits your needs. Once you have decided to begin treatment, additional records will be taken to ensure the best possible outcome.


If you have dental insurance, we will help you to determine the coverage you have available. We ask that you assign your insurance benefit to us. The balance will be arranged for you to pay. We will help in every way we can in filing your claim and handling insurance questions from our office on your behalf.


Our practice accepts assignment of benefits for most PPO insurance plans including: Delta Dental, Aetna, CIGNA, Anthem, Guardian, Humana, GEHA, Met Life, Risk Management, United Concordia, Operating Engineers, and UFCW. Please contact our office if your insurance company is not on the list, or if you have any additional questions regarding insurance benefits.

Health History Form

If you are a new patient to our office, please visit our Health History Form page (under the Patient tab) to download the form. Our Health History form will need to be filled out prior to arriving at our office. You can either print it, fill it out, and bring it with you to your appointment, or email the form back to us at: info@menifeebraces.com. If you forget the form or aren't able to download it, we have the form at the office for you to fill out. Thank you and please contact our office if you have any questions at all.

Regular Appointments

The initial appliance placement appointment usually takes from one to two hours. We would like to see you every 6-8 weeks to make adjustments to your braces or aligners, which usually takes thirty minutes. It is important to keep these appointments, as missing appointments may prolong treatment. We make every effort to be on time for our patients, and we ask that you extend that same courtesy to us.


If you are more than 15 minutes late to your appointment, we may have to reschedule you. If you know a head of time you cannot keep an appointment, please notify us immediately so we can make that appointment time available for another patient.

Wednesday: 6:00AM- 6:00PM

Thursday: 6:00AM- 5:00PM

Friday-Monday: Closed


Closed daily for lunch: 12:00PM- 1:30PM

Treatment Fees

Dr. Erickson believes that cost should not be a barrier to receiving excellent orthodontic treatment.


The cost of treatment depends on the severity of the patient's problem and appliances used. You will be able to discuss fees and payment options before treatment begins. We also accept assignment from most insurance plans ( Click Here To View Accepted Plans ), and file the necessary paperwork to the insurance company. We work hard to make orthodontics affordable.


In an effort to keep orthodontic fees down while maintaining the highest level of professional care, we have established this financial policy:

 

  • If full payment is made at the onset of treatment, we will offer a fee discount. (If you have insurance, we will offer a discount for payment of your portion of the fee that will not be covered by insurance.)
  • To fit your individual needs, financial arrangements can be made to extend your payments over a period of months.
  • For your convenience, we accept payment by cash, check, debit, or credit.
  • In cases of serious delinquency, treatment will be discontinued, although we will make every effort to accommodate patients with temporary difficulties.
  • We also offer a substantial family discount (up to 5%) for multiple family members that have treatment in our office.
